Upon moving in, Jane encounters her first-floor neighbor, Martha Stewart (Duvall), who introduces her to the building’s occupants. Among them is Alice, an enigmatic old woman living on the 4th floor, known for her reclusive nature. The following morning, Jane discovers a note on her door, warning her about the noise she made while moving in. Dismissing the warning, she continues to settle into her new apartment.
As Jane unpacks, another ominous note appears, prompting her to call a locksmith (Bell) after someone tampers with her locks. The locksmith, a peculiar man from the building across, raises Jane’s suspicions, especially after she learns of his questionable past involving a woman in distress.
Jane finds solace in her friendship with Mr. Collins (Pendleton), a kind elderly man who shares a connection with Greg from high school. However, tensions rise when Jane and Greg argue about her decision to live alone. Following a confrontation with her downstairs neighbor, who retaliates by smashing her kitchen tiles, Jane calls the police, only to find herself embroiled in a cycle of noise and retaliation.
Things take a turn for the worse when Jane discovers her apartment is infested with maggots and mice. Desperate for help, she calls an exterminator (Costanzo), who reveals that the infestation may not be a mere coincidence. Jane learns from a local grocer that Alice has stopped ordering groceries, raising her suspicions about the mysterious neighbor.
When Martha chastises Jane for her actions, claiming she disrespects privacy and authority, Jane feels increasingly isolated. The exterminator’s revelation that the mice were likely introduced through a drilled hole in her floor sends her into a panic. Determined to uncover the truth, Jane breaks into the 4th floor, where she discovers a disturbing scene: cages filled with mice, a typewriter, and a wall mapped out with her furniture, all marked with the word “Portcullis.”
As the tension mounts, Jane receives a package containing packing noodles and her own note, now ominously marked “Truce accepted.” The situation escalates when she finds photos of her aunt’s deceased body, prompting her to gather evidence and seek help from the police. However, her plans are thwarted when she is attacked by an unknown assailant, and her evidence is stolen.
After recovering from her injuries, Jane returns to her apartment, only to find the drilled hole has vanished. Greg, concerned for her safety, urges her to leave the apartment and stay with him. Frustrated, Jane confronts her neighbor, demanding answers. Mr. Collins offers her comfort, but Jane soon realizes the horrifying truth: he is the very neighbor terrorizing her.
Knocked unconscious by Mr. Collins, Jane awakens in a room filled with flies and the body of Alice, covered in maggots. In a desperate bid for survival, she fights back, using a crowbar against her captor. However, her escape attempts are thwarted as Mr. Collins ties her door shut and breaks in through the window.
In a climactic showdown, Jane manages to turn the tables on Mr. Collins, using her surroundings to her advantage. With the help of the locksmith, who arrives just in time, Jane fights for her life. In a final act of desperation, she pushes Mr. Collins over the stair railing, leading to his fatal fall.
Months later, Jane is seen in Greg’s apartment, discussing the locksmith with her friend Cheryl (Grdevich). She expresses gratitude for the locksmith’s help, unaware of the dark secrets he harbors. The film concludes with a chilling revelation: the locksmith has been observing Jane and Greg, sketching their lives, and the camera lingers on a painting that connects Greg and Mr. Collins, hinting at a deeper conspiracy.
