Three years after surviving a terrifying encounter with the vampire Jerry Dandrige, Charley Brewster, now 20, has been undergoing psychiatric therapy. He has come to believe that Jerry was merely a serial killer posing as a vampire, and in turn, has convinced himself that vampires do not exist. But little does Charley know, his life is about to take a dark and supernatural turn once again.
Charley, along with his new girlfriend Alex Young, pays a visit to Peter Vincent, the once-renowned vampire killer from “Fright Night.” However, the encounter is far from comforting as Charley finds Peter is now a burnt-out, washed-up version of his former self. While visiting Peter’s apartment, Charley notices a strange occurrence—three large crates are being unloaded from a truck, adding to his unease. As Charley and Alex leave, Charley sees four mysterious individuals entering an elevator, one of whom catches his attention: the alluring Regine. Instantly drawn to her, Charley becomes obsessed, even though he’s trying to suppress the lingering belief in vampires.
Later that evening, Charley’s obsession grows stronger when he kisses Alex, only to hallucinate himself kissing Regine. Distraught, Alex storms off, unknowingly becoming the target of one of Regine’s vampires. While Alex enters her dormitory, another girl is followed and killed by Belle, one of Regine’s followers. As Alex remains oblivious, Louie, another of Regine’s group, tries to stalk her, but his attempt is foiled when she slams the window shut on his hands. Meanwhile, Bozworth, Regine’s creepy, bug-eating servant, mocks Louie and consumes insects, adding a grotesque layer to the unfolding events.
Later that night, Charley is haunted by a disturbing dream in which Regine transforms into a vampire and attacks him. When he discusses this with his psychiatrist, Dr. Harrison, the doctor reassures him that the dream is a natural reaction to his past trauma. Charley tries to move on with his life, bowling per the doctor’s suggestion, and agreeing to accompany Alex to the symphony. However, his plans take a detour when he spots his friend Richie with Regine. Charley’s curiosity leads him to scale a fire escape outside Regine’s apartment, where he witnesses Richie being attacked and drained of blood by Regine and Belle. Horrified, Charley runs to find Peter, and the two arm themselves with crosses to confront Regine’s cult.
At Regine’s party, Charley finds Richie miraculously alive and without any bite marks. Regine makes a dramatic entrance, dancing seductively with Charley, who is now fully captivated by her. She introduces herself as a performance artist, but Peter remains suspicious of her intentions. After witnessing strange behavior, such as people biting each other on the neck and Regine and Belle casting no reflections in a mirror, Peter realizes that Regine is indeed a vampire—Jerry Dandrige’s sister—and she has come to seek revenge on Charley and him.
That night, as Charley falls asleep, Regine visits him in his dreams and bites him on the neck, marking the beginning of his transformation into a vampire. Denying the truth despite the evidence, Charley continues to rationalize the situation. When Peter arrives to warn Charley and Alex, neither of them believe his story, and Peter, frustrated, leaves to prepare for the inevitable confrontation. Meanwhile, Charley starts to show signs of becoming a vampire, developing a sensitivity to garlic and sunlight. He begins to accept that the world of vampires is real after hearing about Richie’s death on the news.
As Charley’s transformation progresses, Alex and Charley attempt to stop Louie, who has been stalking Alex. They confront him in the school library, where Alex successfully incapacitates him by forcing wild roses—harmful to vampires—into his mouth. However, their encounter leads to their arrest by campus security. Meanwhile, Peter, having gone off the deep end, attempts to kill Regine on live television, which results in him being placed in a state hospital, furthering his descent into madness.
Alex eventually bails Charley out of jail, but soon discovers that Charley has been taken by Regine. Along with Dr. Harrison, who turns out to be a vampire himself, Alex navigates through a dangerous series of events, ultimately defeating Dr. Harrison and assuming his identity to free Peter. With Peter’s help, Alex confronts Regine in her lair, rescuing Charley from his slow transformation into a vampire. Together, they fight and kill the vampires who had been assisting Regine—Belle, Louie, Bozworth, and even Richie, who has become an undead minion.
As Regine attempts to flee in her coffin, Charley and Alex thwart her escape by lining it with Communion wafers. After a fierce struggle, Peter finally destroys Regine using sunlight, ending her reign of terror. The remaining vampires are destroyed, and Charley is saved from his dark fate.
In the aftermath of the battle, Charley and Alex reflect on the harrowing events. They jokingly discuss how no one would ever believe their story, and while they ponder whether there are more vampires out there, they agree to move forward with their lives, prepared for whatever may come next. As they embrace, a bat flies away, leaving the door open for more supernatural adventures.
